UnitedHealthcare® offers VisionWise Premier plan for all ages at est. monthly premium of $13.28. The plan covers:- Eye Exam,
- Standard Lenses: Single-Vision, Bifocal, and Trifocal
- Frames, and
- Contacts Lenses. (up to 12-month supply)
- Any age
- Network of providers have discounted service rates. Access network of providers by visiting MyUHCvision.com.
- Non-Network providers have limited insurance coverage.

Open Quote and Sign up here on the UnitedHealtcare® site. Quote and Sign up here
Fill the form and click on View Plans.
Navigate and click on tab Vision, you see plan VisionWise Premier. Click on View more details to see scope of plan.
Click on tabs: Plan Overview, My Doctors, and Standard Benefits to browse over details.
Click on button Add to Cart. Follow screens to click on button Apply and sign up.
Complete and submit application.
